Home Based Care

A tremendous amount of suffering takes place within remote rural communities amongst the sick, disabled and elderly. Adults and children are dying without any dignity under shocking circumstances.
The majority of the patients identified do not have the means to get to the closest clinic or hospital.
It is due to these heart rendering situations that Swa Vana decided together with community leadership to nominate a group of community members to be trained as Home Based Care Volunteers to assist children and adults in the Huntington community who are faced with desperate situations.

Home Based Care Impact of Service

• Swa Vana HBC would like to be a support network to community members who are ill, disabled, frail, old or any other disadvantaged people in need of palliative care.
• The result will be human dignity.
• We have a responsibility towards one another to hold hands and give strength to a community by reaching those who are in urgent need of care because they find themselves alone.
• HBC will strengthen the capacity of the community by not only helping the ill, but supporting the needs of the families who are just as affected.
• HBC will educate and counsel relatives to care for sick family members.
• HBC will work to reduce the stigma of HIV/AIDS and increase awareness through education, thereby attempting to reduce the incidents of HIV/AIDS.
• Assisting the children who are left behind due to the numerous deaths which impact our country enormously due to the pandemic.
• HBC testing awareness campaigns.
